Here’s some important information for transitioning Kaiser Permanente members

What to do BEFORE December 31, 2022

  1. Follow Humana Military’s instructions
    Follow the instructions you received from Humana Military about changing your PCM to a new TRICARE network provider or enrolling in TRICARE Select during TRICARE Open Season. Open Season is November 14 to December 13, 2022. Contact Humana Military at 1-877-414-9950 for more information.
  2. Schedule your Kaiser Permanente appointments
    Schedule any needed appointments with your Kaiser Permanente health care team as soon as possible so your appointments can be completed before the end of the year.
  3. Refill your prescriptions
    Order any needed Kaiser Permanente prescription refills before December 31, 2022, to ensure you have an adequate supply to carry you through this transition. You can easily submit a refill request on kp.org/rxrefill or call the Refill Pharmacy at 770-434-2008. If you’re out of refills, email your provider to request a refill.
  4. Save your medical information
    Download any information you want to save from your kp.org account—such as test results, immunizations, or after visit summaries—before December 31, 2022. After that date, you’ll have limited read-only access to your information until March 1, 2023.
  5. Earn and activate your Wellness Rewards health payment card
    Complete the steps to earn your Wellness Rewards health payment card, up to $400.00 per household. You can use any reward you earn in 2022, or any reward you previously earned but haven’t used, until December 31, 2023. Go to kp.org/tricare/rewards for details.
  6. Respond if we reach out
    If you’re in a chronic conditions program, in treatment for cancer or another serious condition, or pregnant, we’ll be contacting you to help you with this transition. Be sure to respond when we reach out.

Helpful information for AFTER December 31, 2022

Transferring your medical records
Your new doctor can contact Kaiser Permanente directly by phone or FAX to request your medical record, if needed. We will provide your medical record to your new doctor at no cost.

Transferring your prescriptions
If your current prescription was filled in a pharmacy located in a Kaiser Permanente medical facility, you will need to have your prescription transferred to a retail pharmacy that participates in the TRICARE pharmacy network.  Your new pharmacy can contact Kaiser Permanente directly by calling the phone number on your medication bottle, and we will transfer the prescription.

Use your wellness reward
If you earned your wellness reward by completing the necessary steps, don’t forget to use the funds on your health payment card. Visit our wellness rewards program page at kp.org/tricare/rewards for more information.
