The Total Health Assessment starts with a series of questions about your health. Based on your answers, we’ll provide guidance on how you can make small behavior changes that will help you reach your personalized health goals.

Before you start:

  • The Total Health Assessment will open in a new tab of your browser.
  • Your kp.org tab will stay open and signed in so you can use your medical record as needed to answer assessment questions. ​ 
  • After 20 minutes of no activity, you’ll be signed out automatically.​

Make good health a part of your daily habits with our free healthy lifestyle programs.* These personalized, online programs can help you create an action plan to reach your health goals.

Based on your Total Health Assessment results, you’ll find advice, encouragement, and tools that can help you make healthier lifestyle choices. Topics include:

  • Eating healthy
  • Losing weight
  • Moving more
  • Sleeping better
  • Reducing stress
  • Quitting smoking
  • Managing health conditions

You can come back to these programs at any time and track your progress. Plus, you can set new goals and join additional programs.

Schedule convenient phone sessions at times that work for you. Coaching is available at no cost to Kaiser Permanente members, in English and Spanish. No referral is needed.*
