What is the Gwinnett County Employee Wellness Center?

The Gwinnett County Employee Wellness Center is a designated space within the Gwinnett County government property for use by Gwinnett County employees and their dependents ages 3+. It is jointly managed and operated by Kaiser Permanente and Crossover Health. Kaiser Permanente members will see Dr. Hall as their primary care doctor for care.

What disease management services are offered at the Gwinnett County Employee Wellness Center?

The following chronic disease management services are offered:

  • Diabetes
  • Asthma treatment
  • COPD

  • Hypertension
    (high blood pressure)
  • Thyroid conditions
  • Weight management

What services will Kaiser Permanente offer at the Gwinnett County Employee Wellness Center?

Kaiser Permanente will provide primary care services for Gwinnett County members and their families:

Can covered dependents receive care at the Gwinnett County Employee Wellness Center?

Yes, covered dependents age 3 and above can receive care at the Gwinnett County Employee Wellness Center.

Will I be seen by my Kaiser Permanente primary care doctor?

Dr. Hall is available to serve as your dedicated primary care doctor. He will have access to your electronic health record.

Will I have to change my Kaiser Permanente primary care doctor in order to be seen at the Gwinnett County Employee Wellness Center?

No, you are not required to change your primary care doctor in order to be seen at the Wellness Center. However, Dr. Hall is available to select as your primary care doctor. The Gwinnett County Employee Wellness Center is an alternative location for you to receive primary care.

Will my co-pay change?

There will be a $20 co-pay for medical visits. You can get preventive screenings such as, biometric screenings, coaching, physicals, A1C tests, blood pressure checks, most immunizations and disease management usually at no cost. You will continue to pay applicable co-pays for services received in one of our Kaiser Permanente medical centers.

How can I schedule an appointment?

You can schedule your appointment by calling the Gwinnett County Employee Wellness Center at 678-377-4080. This line is available Monday-Friday, 7:00am – 7:00pm. Follow up appointments can be made via phone or online at kp.org.

If a Kaiser Permanente provider is not available, can I see a Crossover Health provider?

No. Kaiser Permanente members can only be seen by a Kaiser Permanente provider. If you need care during the hours a Kaiser Permanente provider is not available, we encourage you to make an appointment at another Kaiser Permanente medical facility or make an appointment online on kp.org. In the event of an urgent matter, visit the 24/7 Advanced Urgent Care at any of our three Comprehensive Medical Centers: TownPark, Southwood, or Gwinnett.

Can I complete any of my activities for the Wellness Program at the Gwinnett County Employee Wellness Center?

Yes. You can complete your biometric screening, immunizations, wellness coaching, physicals, and make a Wellness Plan.
