Conditions

As a cardiac patient you may have been diagnosed with one or more of the following health conditions:

  • High blood pressure
  • High blood sugar
  • Excess body fat around the waist
  • Abnormal cholesterol or triglyceride levels

These conditions can increase your risk of complications with your heart disease.  If you have been diagnosed with one of these conditions, you have a greater risk of serious disease.  Having three or more of these factors will result in the diagnosis of a condition called metabolic syndrome, and this increases your risk of further health complications.

Metabolic syndrome is increasingly common, and up to one-third of U.S. adults have it. If you have metabolic syndrome or any of its components, aggressive lifestyle changes can delay or even prevent the development of serious health problems.

As a member of the VCR program, you and your Case Manager will review any conditions you have been diagnosed with, as well as teach you how to monitor and control your corresponding vital signs as prescribed by your Cardiologist.
