Important Medicare Health Plan Contacts

Non-emergency transportation

No cost rides for nonurgent medical appointments at Kaiser Permanente medical centers and contracted facilities.

Rides must be scheduled at least two hours before your pick-up time. You must cancel rides at least 2 hours before the scheduled pick-up time.

If not, the ride will be deducted from your annual ride balance. You can also call to check your ride balance.

Healthy Food Card Benefit

Allowance helping with overall health for members with at least one chronic condition.

Must have a visit with your PCP first. Review your Summary of Benefits for a list of qualifying conditions. Benefits are part of a special supplemental program for the chronically ill.

Not all members qualify and members who receive coverage through their employer are not eligible. Visit mybenefitscenter.com and download the OTC Network mobile app to find participating stores, search for eligible items, and check your card balance.

Vision Services & Hardware

Routine eye exam included and an allowance every two years for glasses and contacts.

Get your vision hardware like contacts and glasses at locations such as For Eyes, My Eye Doctor, and many more locations.
