Diabetes Prevention & Management Programs
Diabetes Prevention Program Recognized by CDC
In our Diabetes Prevention Program, patients with pre-diabetes have the opportunity over the course of one year to attend classes (in-person and virtually) and work with trained lifestyle coaches. In class, patients learn about dietary changes, behaviors, and physical activity that can allow weight control to be sustainable.
We also offer a series of diabetes management classes that help members understand diabetes basics, what they should eat, how to manage health risks, and how to manage their conditions long term. Routine Screenings
Members considered at risk for diabetes are screened for prediabetes or type 2 diabetes.
Risk factors for diabetes include:
- Age 45 or older
- Overweight, with a body mass index (BMI) higher than 25
- Women with a history of gestational diabetes.
Other warning signs can include excessive thirst or urination, blurry vision, or unintended weight loss.
